What are the causes of back pain in pregnant women?

Back pain is a very common symptom among pregnant women, especially during the second and third trimesters. It is estimated that about 50% to 70% of pregnant women experience varying degrees of back pain during pregnancy. This proportion can vary depending on different studies, populations, and assessment criteria.
The incidence of back pain is influenced by multiple factors, including weight gain, postural changes, structural and physiological changes in the pelvis, and hormonal changes leading to joint and ligament laxity. Additionally, if a pregnant woman has a history of back pain before pregnancy, her chances of experiencing back pain during pregnancy may be higher.
Back pain during pregnancy is quite common and can have several possible causes:
- Hormonal changes: During pregnancy, the body produces a hormone called relaxin, which loosens the ligaments in the pelvic area to prepare for childbirth. This relaxation can lead to instability in the pelvic support structures, causing back pain.
- Shift in center of gravity: As the baby grows, a pregnant woman's center of gravity shifts forward, which can increase pressure on the lower back, leading to changes in posture and overstretching of back muscles.
- Increased weight: The increase in body weight during pregnancy puts extra pressure on the spine, especially the lower back.
- Stretching and weakening of abdominal muscles: As the uterus expands, abdominal muscles stretch, reducing their supportive strength, which increases pressure on the lower back.
- Pelvic changes: During pregnancy, the pelvis may widen in preparation for childbirth, which can affect pelvic stability and support for the spine.
- Changes in spinal curvature: To balance the increased weight in front, a pregnant woman's spine may increase its natural lordotic curve (lumbar lordosis), which can lead to tension in the lower back muscles and ligaments.
- Sleep posture: As pregnancy progresses, finding a comfortable sleep position can become increasingly difficult, and poor sleep posture may cause or exacerbate back pain.
- Stress and emotional factors: Stress and emotional fluctuations can also affect body tension, leading to muscle tightness and pain.
